本合同包专门面向中小企业采购，投标人应为中型企业或小型、微型企业/监狱企业/残疾人福利性单位。投标人须提供《中小企业声明函》，且中小企业的划分标准所属行业为零售业；？
投标人为监狱企业的，应提供监狱企业的证明文件；投标人为残疾人福利性单位的，应提供《残疾人福利性单位声明函》。监狱企业/残疾人福利性单位均视同小型、微型企业。落实的政府采购？政策文件如下：？

3.本项目的特定资格要求：

合同包1(窗帘及床上用品)特定资格要求如下:

（1）营业执照等主体资格证明文件：提供有效存续的企业营业执照（正/副本）及？2021？年度企业年度报告/事业单位法人证书/专业服务机构执业许可证/民办非企业单位登记证书。？
（2）财务状况报告：2021？年度财务状况良好，无偷税漏税、不良资产等问题（需提供经会计事务所或审计部门出具的财务审计报告），公司成立不足一年的提供自成立以来的财务报表（至少包含资产负债表、利润表、现金流量表）或公司基本账户开户行出具自成立以来的资信证明及基本账户开户证明。？
（3）税收缴纳证明：提供递交响应文件截止之日前一年内任意一个月的依法缴纳税收的相关凭据（时间以税款所属日期为准），凭据应有税务机关或代收机关的公章或业务专用章。依法免税或无须缴纳税收的供应商，应提供相应证明文件。？
（4）社保缴纳证明：提供递交响应文件截止之日前一年内任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明。依法不需要缴纳社会保障资金的供应商应提供相关证明文件。？
（5）履行合同能力证明：提供具有履行本项目合同所必需的设备和专业技术能力的证明资料或承诺书。？
（6）信用记录：提供《书面声明函》（按给定格式填写），供应商不得为“信用中国”网站记录的严重失信主体、重大税收违法失信主体、政府采购严重违法失信行为记录名单，不得为“中国政府采购网”的政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的企业。
（7）身份证明书：法定代表人参加谈判的，提供法定代表人证明书；法定代表人授权他人参加谈判的，提供法定代表人授权委托书。？
（8）信用承诺书：提供《榆林市政府采购货物类项目供应商信用承诺书》（按给定格式填写）。
